About the Course

This course helps providers strengthen documentation across notes, reviews, reports, planning records, and support-related follow-up. Learners practice identifying the details that make documentation useful: what was happening, what support was needed, what action was taken, how the person responded, and what the team may need to review. All examples are fictional and designed for practice. The course emphasizes dignity, confidentiality, objective wording, bias reduction, and documentation that can be understood by the intended reader.
